READ THE FOLLOWING CAREFULLY as it is important that you come to register with all the proper paperwork.
Students with the exception of students in public and private day schools need to register
at their home school. Pre-K, Early Childhood and students attending public and private day
schools need to register at LakeView School.

ALL PUPILS NEW AND RETURNING, Kindergarten—Grade 8
According to the board policy adopted in June 1994, the Board of Education has directed
the administration to request verification of residence for K-8 students during
registration. The district re-registers each student in grades K-8 each year.
Parent/guardians must show proof of residency at the time of registration each year
by providing 1) a current lease or mortgage payment; 2) a current gas or electric bill;
and 3) a valid photo ID for the parent/guardian that includes full legal name with
middle initial and parent/guardian date of birth.
A person who enrolls or attempts to enroll in this school district on a tuition-free
basis a student known by that person to be a non-resident of the District is guilty
of a Class C misdemeanor, except as defined by State law (105 ILCS 5/10-20, 12b(e).
If a student is determined to be a non-resident of a District for whom tuition must
be charged, the person(s) enrolling the student are liable for non-resident tuition
from the date the student began attending a District school as a non-resident. The
District residency officer is responsible for checking that the children in our
District are legally living in the District.
Students transferring from other Illinois public schools must present copy of their
Illinois Student State Transfer Form at the time of registration. In addition,
special education students should be registered at their home school. Special
education students that attend an out-of-district school should register at Lakeview.
Student Instructional Material/Supply Fees
The Board of Education initiated a Material/Supply Fee to partially cover the cost
of supplies, instructional materials, and workbooks. The Instructional Materials and
Supply
Fees are $48.00 for grades K-6 and $60.00 for grades 7-8.
Fee Waivers
Parents may be eligible for a waiver of student fees under the following circumstances:
1. If the family receives public aid
2. The family income is under the amount set in federal guidelines for free or reduced price lunches
3. The family can otherwise establish that they cannot afford to pay these fees
Applications for requesting a waiver of fees must be submitted to the building principal. Applications
are available in the principal’s office or online at www.zion6.com. The use of false information to
obtain a waiver is a felony under Illinois law. Any inquiries should be directed to the principal of
the school in which students are enrolled. Qualifying students are not exempt from charges for lost
or damaged books, locks, materials, supplies, and equipment.
PUPILS NEW TO ZION DISTRICT 6
Illinois state law allows children to attend kindergarten if they are age five on or before September
1 of that same year. Those who will not be five years old until September 2 or later must wait until
the following school year to begin kindergarten. A birth certificate is required for kindergarten
registration.
Early Childhood and Pre-Kindergarten Registration
Registration for students enrolled in the Early Childhood Program and in the Pre-Kindergarten
Program occurs at Lakeview School during normal District 6 registration dates. Children who are
age three to five may qualify for the Pre-Kindergarten Program. Call 847-872-5455, ext. 121 for

Transfer Students
Students transferring from another Illinois public school must submit a copy of the Illinois
State Student Transfer Form at the time of their registration. Registration and admission will
not occur until the State Transfer Form is presented. Parents of students transferring into
Zion Elementary School District 6 from an out-of-state public school district must certify in
writing that the student is not currently serving a suspension or an expulsion imposed by the
school from which the student is transferring.
A student transferring from an Illinois or out-of-state public school district shall not be
permitted to attend class if he or she has been suspended or expelled because of possessing
of a weapon (as defined in the Gun Free Schools Act), knowingly possessing, selling, or
delivering a controlled substance or cannabis, or for battering a staff member of the school.
In addition, if the period of suspension or expulsion has not expired at the time the
student attempts to transfer into any Zion elementary school, the student shall not be
permitted to attend class until the student has served the entire period of the suspension
or expulsion that was imposed by the school from which the student is transferring.
PUPIL HEALTH RECORDS
Preschool Health Needs:
New physical if the student is new to Preschool, this must include Lead info and all shot
records. Due by October 15th of 2008.
Kindergarten Health Needs:
New Physical - done within the last year from the day school starts, the physical must
include Diabetic screening and BMI.
All shot information, including the 2nd MMR, and last DPT and Polio. Lead info is required,
and TB test is recommended.
Dental exam done, we will accept any exam done after November 15th of 2007 (Paperwork due
by May 15th 2009).
New this year, is the eye exam, this must be done by an eye doctor and on the proper form,
which you can get at the doctor.
At registration all forms will be available if needed in the Kindergarten Packet.
Deadline for physical, eye exam and shot information is due by October 15th of 2008.
Second Grade Health Needs:
Dental exam done, we will accept any exam done after November 15th of 2007 and the deadline
for the paperwork is due by May 15th of 2009.
Sixth Grade Health Needs: New Law this year
Dental exam done. New Physical including the shot records, a Diabetic Screening and BMI.
If your 6th grade student got a new physical in 5th grade, this will be accepted for this
year only. This is due by October 15th of 2008.
Sports Physicals must be renewed every school year, not every calendar year.
So if you think that your child will be involved in any sports this new school year, they
must have a sports physical done. We will be hosting a physician who will be coming to
Central Jr. High to do a physical for $25.00 cash on Wed. August 20th 2008. You can sign
up for this exam at registration. See the health clerk at your school.
